Urgent patient update

Dear Patients, due to pre-planned annual leave and unexpected sickness within our clinical team, we will need to prioritise urgent appointments this week.

We kindly ask that, where possible, any routine or minor concerns are postponed until next week.

This will help us ensure that patients requiring urgent care can be seen promptly. Thank you for your understanding and cooperation.

Patient Health Questionnaire (PHQ-9)

Use this service to submit a routine review of your depression.

You can use this service if you:

  • are registered at the surgery
  • have already seen a doctor, nurse or other healthcare professional and have been invited to do so

Before you start

Call 999 or go to A&E now if:

  • someone’s life is at risk – for example they have seriously injured themselves or taken an overdose
  • you do not feel you can keep yourself or someone else safe

A mental health emergency should be taken as seriously as a medical emergency. If you are having a mental health crisis or are considering self-harm, please seek help immediately.

  • if applicable, the details of the person you are completing the form on behalf of
