Due to staff sickness in our admin team, phone waiting times may be longer than usual today as we are also running our Patient Digital Support Day.

Change your personal details

Use this service to change your address, name or phone number. It is important that we have up to date contact details in case we need to contact you.

You can use this service if you:

  • are registered at the surgery

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
  • evidence if your name has changed due to marriage or by deed poll
