We can only accept samples in the approved sample bottles, we provide these free of charge from reception, based on a clinicians request.
It is important to complete the patients details on the bottle:
Name / Date of Birth / Telephone Number
Please also complete the form that comes with the bottle in the plastic bag.
Please return the sample bottle and the form, in the clear plastic bag provided.
The patient advisors on the desk have been advised NOT to accept samples that are not in the approved bottles.
Reception is unmanned between 1pm and 2pm, so you will be unable to hand samples in at this time.
